If you are unable to hire counsel and wish to screen for a public defender, complete the Public Defender Indigency Screening Form (PDF) and bring it to your hearing. Please note, public defenders are for criminal matters only, not civil infractions. If you are appointed, please wait one week before reaching out to the assigned firm.

If you have a complaint or comment about your public defender please fill out the form below and return it to the city of Lakewood at the address listed on the form. Once received, city personnel are required to respond. Once filed with the city the completed form is a public record and potentially eligible for release.
